From Valentines 2004, this card represents a pictorial variety of chocolates. When issued, the card was placed in a small box instead of a card sleeve (hence, Box of Chocolates).
The sleeve (or box in this case) is shown below before it was assembled. This combination also had a reference guide that completes the issue.
